MELD Score (Model for End-Stage Liver Disease) is used to measure the seriousness of liver damage for patients with end-stage liver disease and the likelihood of their survival. If you are a candidate for Liver Transplant, MELD determines how urgently you need a transplant.

A MELD score is a number that goes from 6 to 40 and is based on results of several lab tests. The higher the number, the more pressing your case is.

Making a MELD Score

Your MELD score depends on results from few lab tests, including your:

  • Creatinine level, which is identified with how well your kidneys are functioning
  • Bilirubin level, which shows how well your liver clears a substance called bile
  • INR (worldwide standardized proportion), which reflects how well your liver makes factors required for blood clumps
  • Serum sodium level, which shows the grouping of sodium in your blood

How to calculate meld score

It is calculated according to the following formula:

MELD = 3.78×ln[serum bilirubin (mg/dL)] + 11.2×ln[INR] + 9.57×ln[serum creatinine (mg/dL)] + 6.43/p>

MELD scores are reported as whole numbers, so the result of the equation above is rounded.

  • If the patient has been dialyzed twice within the last 7 days, then the value for serum creatinine used should be 4.0 mg/dL
  • Any value less than one is given a value of 1 (i.e. if bilirubin is 0.8 a value of 1.0 is used) to prevent subtraction from any of the three factors, since the natural logarithm of a positive number below 1 (greater than 0 and less than 1) yields a negative value.

How to interpret the MELD Score

  • 40 or more — 71.3% observed mortality
  • 30–39 — 52.6% observed mortality
  • 20–29 — 19.6% observed mortality
  • 10–19 — 6.0% observed mortality
  • <9 — 1.9% observed mortality

How frequently can MELD Score be calculated?

You could get your MELD score recalculated occasionally. Your ailment, treatment, and past MELD score all influences how frequently MELD Score needs to be updated.

Typical MELD Score and how often MELD Score is recalculated

  • Under 10: It will be recalculated once every year.
  • 11-18: It will be recalculated like every 3 months.
  • 19-24: It will be recalculated once every month.
  • 25 or higher: It will be recalculated each week.

MELD Isn’t Everything

Your MELD score is a prime indicator in determining how urgently you need a transplant. But, it isn’t the only factor. Other considerations include:

  • Your blood type
  • Your body size relative to the donor’s
  • Geographical considerations (i.e. how close you are to the donor liver)
  • Current supply of, and demand for, deceased donor livers
